Instructions On How To Submit Signature:

1) Sign your name (3) times on plain white paper using black ink.

2) Scan or take a picture of your signature with your phone. Helpful hint when using your phone – Hold the signature up in front of you and take the picture. Taking the picture of the signature on the desk may lead to shadows.
